Community Engagement Specialist, Alpine County

Minimum of 1-3 years of experience in healthcare, community outreach, or a related field. Experience in engaging with diverse communities and providing culturally sensitive communication. Fluent in English and Spanish (verbal and written). Strong interpersonal and communication skills. Detail-oriented with excellent organizational skills. Comfortable with technology and proficient in using electronic health records (Athena experience preferred). Ability to work a flexible schedule, including evenings and weekends. Access to reliable transportation for in-person outreach and events.

Conduct in-person outreach attempts for members. Complete Athena documentation for outreach attempts. Schedule intake appointments and assist with form completion. Attend outreach events and represent Zócalo Health. Build and maintain relationships with community partners. Convert leads to patients using internal documentation systems. Document all outreach attempts and outcomes in Athena. Assist in sending patient mailers and follow-up calls. Represent Zócalo Health at community coalition meetings.
