Requirements:
Live in West Virginia or bordering state Experience working with Community Health Centers Graduate of an accredited college or university in healthcare, health technology or information systems-related programs (or significant relevant work experience) Minimum 6-8+ years of experience with value-based care concepts (triple aim, risk stratification, accountable care) Minimum 6-8+ years of experience with relevant competencies (practice management, project management, account management, analytical problem solving, strategic insight using healthcare analytical skills) Ability to perform project management and account management best practices Results-oriented with the ability to prioritize and manage multiple projects simultaneously Excellent computer skills using Excel and willingness to learn additional software applications Skilled with using data to drive planning and evaluation of success Ability to travel 10-25% of the time
Responsibilities:
Assist ED/MP on market strategy implementation. Manage practice transformation priorities aligned to ACO strategic plans and practice transformation plans. Conduct regular practice calls and visits. Support and lead board meetings as appropriate. Ensure full implementation of all net-new practice programs and compliance requirements. Manage staff, assist in new staff training and onboarding. Establish goals for and review KPIs with each team member. Support professional development of PTS. Prioritize and cascade company strategy autonomously and professionally. Demonstrate effective proactive coaching and staffing models. Identify and address areas of opportunities related to direct reports and timely communicate opportunities and resources. Provide thought leadership and feedback to improve and innovate Aledade's offerings.
