Remote Licensed Practical Nurse- CA License Required

Active multi-state compact Practical Nurse License. Active CA state LPN/LVN license required. 5+ years of clinical experience as a Licensed Practical Nurse. Consistently stable Wi-Fi with strong upload/download speeds for remote work. Highly skilled in behavioral-based coaching. Excellent clinical acumen and competency in patient assessment. Exceptional written, verbal, and interpersonal communication skills. Prior experience working in a high-growth startup environment. Experience in patient education for chronic conditions. Experience supporting patients remotely is a plus.

Support patient healthcare delivery under direction of Cadence Nurse Practitioner. Provide one-on-one coaching and support to patients managing chronic conditions. Conduct comprehensive assessments of patients' health status and lifestyle. Help patients execute personalized care plans focusing on behavior modification and self-management. Monitor patient progress and adherence to treatment plans. Educate patients on disease management, medication adherence, and prevention.
