Requirements
- Bring 4+ years of field-based relationship or business development experience.
- Ideally have experience within the Colorado CMA landscape.
- Have a deep understanding of Medicaid, disability services, or home-based care.
- Be familiar with Colorado’s case management agency landscape.
- Understand how Case Management Agencies operate and what case managers need to confidently place a client.
- Use a consultative, relationship-driven approach to partner education and development.
Responsibilities
- Grow, foster, and manage relationships with case management agencies on the Front Range, supporting partnerships from outreach and discovery through referral management.
- Target, map, and build relationships with key stakeholders and decision makers at independent Case Management Agencies.
- Educate partners about who fits Abby Care’s programs, how to initiate a referral, and the value of the model for families.
- Open relationships with CMA leadership to drive targeted referral volume and support growth targets.
- Prioritize face-to-face meetings, community networking, and regular partner check-ins.
- Track partner interactions, account statuses, and referral conversion metrics in the CRM.
