Requirements
- Hold board certification in family medicine, internal medicine, or another relevant specialty.
- Have a DEA license and HAP.
- Bring strong clinical skills and organizational skills.
- Balance multiple tasks and work under pressure while keeping communications and documentation organized.
- Use listening and empathy in patient interactions.
- Be comfortable working with elderly patients and individuals undergoing Social Determinants of Health.
- Be able to learn new systems and software quickly.
Responsibilities
- Learn Solace systems, tools, technology, partners, and expectations while providing clinical expertise in each interaction.
- Identify and prioritize patients’ needs and maintain a streamlined care continuum.
- Oversee patient care plans with healthcare advocates to address social determinants of health, including food resources, transportation access, and support at home.
