Requirements
- Reside in Mississippi.
- Hold a DEA license.
- Have HAP.
- Be board certified in family medicine, internal medicine, or another relevant specialty.
- Bring strong clinical and organizational skills.
- Balance multiple tasks and work under pressure while maintaining organized communications and documentation.
- Have strong communication skills, including listening and empathy in patient interactions.
- Be comfortable working with elderly patients and individuals experiencing social determinants of health.
- Be able to quickly learn new systems and software.
- Applicants must be based in the United States.
Responsibilities
- Learn Solace systems, tools, technology, partners, and expectations.
- Provide clinical expertise in patient interactions.
- Identify and prioritize patients’ needs.
- Maintain a streamlined care continuum.
- Oversee patient care plans in partnership with healthcare advocates.
- Address social determinants of health, including food resources, transportation access, and support at home.
- Collaborate with healthcare advocates to help patients achieve their health goals.
