Requirements
- Be board certified in family medicine, internal medicine, or another relevant specialty.
- Have a DEA license and HAP.
- Bring strong clinical skills.
- Be able to balance multiple tasks and work under pressure while maintaining organized communications and documentation.
- Demonstrate listening and empathy in patient interactions.
- Be comfortable working with elderly patients and individuals experiencing social determinants of health.
- Be able to quickly learn new systems and software.
- Be based in Rhode Island.
Responsibilities
- Learn Solace systems, tools, technology, partners, and expectations while providing clinical expertise in each interaction.
- Identify and prioritize patients’ needs.
- Maintain a streamlined care continuum.
- Oversee patient care plans in partnership with healthcare advocates.
- Address social determinants of health, including food resources, transportation access, and support at home.
- Contribute to the development of a new patient care program.
