Job Details

Requirements

  • Be board-certified in family medicine, internal medicine, or another relevant specialty.
  • Have a DEA license and HAP.
  • Bring strong clinical skills and organizational skills.
  • Balance multiple tasks and work under pressure while maintaining organized communications and documentation.
  • Listen and show empathy in patient interactions.
  • Be comfortable working with elderly patients and individuals experiencing social determinants of health.
  • Learn new systems and software quickly.

Responsibilities

  • Learn Solace systems, tools, technology, partners, and expectations while applying clinical expertise in each interaction.
  • Identify and prioritize patients’ needs.
  • Maintain a streamlined care continuum.
  • Oversee patient care plans in partnership with healthcare advocates.
  • Address social determinants of health, including food resources, transportation access, and support at home.
