Requirements
- Hold a compact RN license or be willing to obtain compact licensing.
- Use patient assessment skills to provide effective care management remotely.
- Be able to support patients using phone calls and texting tools.
- Build meaningful relationships with patients who would benefit from additional healthcare support at home.
- Demonstrate strong communication and documentation skills.
- Cardiology experience is preferred.
- Remote care management experience is preferred.
Responsibilities
- Provide remote care management support for CardioOne’s virtual care services.
- Develop and manage disease-specific care plans for patients between office visits.
- Reconcile medications and provide patient education and coaching.
- Document care and communicate with clinicians.
- Respond to critical measurement escalations and coordinate changes in care.
- Support assigned LPN Care Coordinators and assist with onboarding new team members.
- Assist the Manager of Virtual Care Services with clinical operations tasks as needed.
