Requirements
- Hold a bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
- Have 3+ years of case management experience; experience with developmental disabilities or Medicaid waivers is preferred.
- Know Indiana waiver programs (HCBS), Medicaid regulations, and compliance standards.
- Be proficient with EHR or case management software.
- Have excellent communication and organizational skills.
- Hold a valid driver’s license and have reliable transportation.
- Have experience with person-centered and trauma-informed care.
- Be able to manage multiple caseloads, prioritize, and meet deadlines.
- Commit to ethical practice and confidentiality.
Responsibilities
- Assess clients’ needs, strengths, and goals to determine eligibility.
- Develop and regularly review Individualized Service Plans (ISPs).
- Coordinate multidisciplinary care and maintain communication with stakeholders.
- Ensure documentation is accurate and compliant.
- Monitor progress and adapt plans as needed.
- Uphold Medicaid waiver program requirements.
- Advocate for clients within healthcare and community systems.
- Participate in training and quality improvement initiatives.
- Collaborate with providers to deliver holistic support.
