Requirements
- Be a Registered Nurse with 2 years of in-home case or care management experience.
- Have experience working with the adult population.
- Know community health and social service agencies and other community resources.
- Be able to travel to member residences within 30 to 40 miles.
- Have a valid driver’s license, car insurance and access to an automobile.
- Be able to work with minimal supervision within the role and scope.
- Be able to use electronic information applications, including electronic medical records.
- Have intermediate to advanced computer skills, including Microsoft Word, Outlook and Excel.
- Have excellent keyboard and web navigation skills.
- Be able to work a flexible schedule of at least 40 hours per week.
- Have a separate home-office room with a locked door and high-speed DSL or cable internet; satellite internet is not allowed.
- Preferred: BSN, 3–5 years of in-home assessment and care coordination experience, health promotion or wellness coaching experience, managed care experience, Case Management certification, and Motivational Interviewing certification or knowledge.
- Preferred: Bilingual — English, Spanish.
Responsibilities
- Assess and evaluate members’ needs and requirements to support optimal wellness.
- Guide members and families toward appropriate care and wellbeing resources.
- Use strategies to manage members’ physical, environmental and psychosocial health issues.
- Identify and resolve barriers that hinder effective care.
- Monitor patient care through assessments and evaluations to track progress toward desired outcomes.
- Create member care plans when needed.
