- At least 3 years of case management experience preferred. Candidates with 6+ years of RN experience, including at least 3 years in case management, may be considered in lieu of the degree.

Requirements
- Hold an active, unrestricted Registered Nurse (RN) license.
- Have a bachelor’s degree; candidates with 6+ years of RN experience, including at least 3 years in case management, may be considered in lieu of the degree.
- Have at least 3 years of case management experience; this is preferred.
- Certified Case Manager (CCM) certification is preferred.
- Experience working in a dialysis or transplant center is preferred.
- Familiarity with Medicare and Medicaid is preferred.
- Have patient advocacy skills and the ability to foster collaboration, positivity, and motivation.
- Have verbal and written communication skills to engage with patients, families, providers, and interdisciplinary teams.
- Have strategic thinking, planning, organization, and problem-solving abilities.
- Be able to adapt and remain effective in a challenging environment with changing priorities.
- Be comfortable using technology and multiple digital applications in a remote setting.
- Be able to work virtually across Pacific and Mountain time zones.
Responsibilities
- Manage a caseload of members with varying levels of clinical complexity and conduct follow-up according to established policies and care cadences.
- Conduct inbound and outbound calls to engage members and provide education and care navigation.
- Educate patients about kidney health, related conditions, dialysis options, transplant pathways, and renal replacement therapy.
- Complete health and social determinants of health screenings, medication reconciliation, care plans, and documentation.
- Advocate for patients by identifying and resolving concerns, connecting them with community resources, and supporting self-management.
- Coordinate care with patients, families, physicians, and ancillary healthcare providers as part of an interdisciplinary team.
- Communicate patient needs to providers and support patients and families in taking an active role in care.
- Maintain accurate documentation in care management systems in compliance with HIPAA, regulatory requirements, operational procedures, and security policies.
- Track performance metrics and key performance indicators using tools such as Excel, OneNote, Outlook, and Word.
- Provide support across time zones, including after-hours coverage when required by business needs.
