Requirements
- Hold an active, unrestricted RN license.
- Have a bachelor’s degree; 6+ years of RN experience, including 3+ years in case management, may be considered in lieu of the degree.
- Three years of case management experience is preferred.
- CCM certification is preferred.
- Dialysis center or transplant center experience is preferred.
- Experience with Medicare and Medicaid is preferred.
- Use Excel, OneNote, Outlook, and Word.
- Support after-hours needs and various time zones based on business need.
Responsibilities
- Manage a caseload of members with mixed acuity and document member data in HealthMap’s Compass care management system.
- Complete member health and social determinants of health screenings, medication reconciliation, and member-centric care plans.
- Educate members about kidney health, related comorbid conditions, dialysis types, and transplant options.
- Engage members in HealthMap’s Kidney Health Program and follow up according to complexity, policy, and cadence.
- Advocate for patients by responding to concerns and helping resolve barriers to care.
- Coordinate care as part of an interdisciplinary team with patients, support networks, physicians, and ancillary providers.
- Use community resources and programs in care planning and notify providers of identified patient needs.
- Navigate Excel, OneNote, Outlook, and Word, and support after-hours needs across various time zones.
