Requirements
- Hold a master's degree in Social Work, Counseling, or a related human services field.
- Hold a Licensed Clinical Social Worker (LCSW) license.
- Have 2-3 years of behavioral health experience; this is preferred.
- Demonstrate assessment, communication, crisis intervention, and care coordination skills.
- Be proficient with Microsoft Office and electronic medical records.
- Work full-time Monday-Friday, 8:00 AM-4:30 PM; the position has no weekends or on-call responsibilities.
Responsibilities
- Conduct behavioral health assessments and develop individualized treatment plans.
- Provide individual, family, and crisis intervention services.
- Coordinate care with physicians, nursing staff, therapists, and community resources.
- Support patients and families with education, resource navigation, and discharge planning.
- Advocate for patients and encourage active participation in treatment goals.
- Monitor patient progress and adjust care plans as needed.
- Maintain accurate and timely clinical documentation in Epic.
- Collaborate with internal and external partners to ensure continuity of care.
