- A minimum of five years of RN experience is required.
Requirements
- Hold an RN license in good standing and be willing to obtain licensure in other states.
- Have a minimum of five years of RN experience.
- Be willing to work PST hours and flex for evenings and weekends when member availability is limited.
- Have experience developing care plans for individuals with chronic conditions and/or chronic kidney disease; this is strongly preferred.
- Have a BSN; this is highly preferred.
- Hold a CDE or CDCES; this is highly preferred.
- Have experience working 100% remotely as a nurse; this is highly preferred.
- Be able and willing to travel occasionally, including overnight stays for corporate gatherings, conferences, and health fairs.
- Be able to attend and professionally engage in video meetings.
- Be proficient with computers, peripheral equipment, applications and systems, including Microsoft products.
- Have reliable internet and power and a designated work area with minimal interruptions.
Responsibilities
- Establish and maintain contact with assigned and active members by video, phone, text, and email in English.
- Obtain and use clinical information to develop individualized member care plans and clinician-centered care plans per NCQA guidelines.
- Assess members’ health status and care coordination needs.
- Collaborate with members’ health care providers as needed.
- Use motivational interviewing techniques, individualized care plans, and the Triple Aim framework to support members in reaching their health goals.
- Explain program benefits to new or inactive members with chronic kidney disease or at risk for it.
- Document, request, and send member information in accordance with HIPAA compliance.
- Meet individual performance metrics supporting member engagement and retention.
