Requirements
- Hold a Registered Nurse (RN) license in good standing.

- Bring clinical and organizational skills to balance multiple tasks and work under pressure.
- Be able to learn new systems quickly and efficiently.
- Demonstrate empathy and a drive to advocate for patients.
Responsibilities
- Learn Solace's proprietary system, tools, technology, and expectations.
- Build trusting relationships with patients through listening and empathy.
- Identify and prioritize patients’ needs.
- Build care plans that support consistent action over time.
- Develop comprehensive patient care plans that address social determinants of health, including food resources, transportation access, and support at home.
