Requirements
- Hold a current LPN or LVN license in good standing.
- Have a track record of working with patients across different backgrounds and communities.
- Bring a patient-centered approach rooted in caring for people and advocating for them.
- Stay organized while managing multiple priorities and maintain steadiness when the pace picks up.
- Become comfortable quickly with unfamiliar systems.
- Take ownership of tasks and handle them without waiting for a nudge.
Responsibilities
- Use your LPN/LVN clinical background and become fluent in Solace's proprietary platform, tools, and standards.
- Build trusting patient relationships through empathy and attentive listening.
- Assess each patient's situation, prioritize needs, and shape a care plan that supports progress over time.
- Design care plans that address clinical needs and everyday barriers, including food access, transportation to appointments, and support in the home.
- Support Medicare members remotely across the full arc of their care.
