Requirements
- Bachelor’s degree in Pharmacy (PharmD preferred).
- Active pharmacist licensure in Illinois or California in good standing.
- Willingness to obtain additional pharmacist licenses in Iowa, Nebraska, and Hawaii as needed.
- At least 1 year of hospital pharmacy experience, ideally in a clinical or acute-care environment.
- Strong clinical knowledge regarding medication safety, dosing, interactions, and therapeutic appropriateness.
- Comfort working across multiple pharmacy information systems and remote clinical workflows.
- Willingness and availability to work overnight shifts, weekends, holidays, and a 7-on/7-off schedule.
- Reliable home internet connection (cable broadband or fiber optic) with minimum speeds of 10 Mbps down/5 Mbps up.
- Ability to maintain all required professional credentials and licenses throughout employment.
Responsibilities
- Perform remote medication order entry, order verification, and drug verification across multiple hospital environments.
- Complete transitions-of-care medication reconciliation and navigate multiple pharmacy information systems.
- Provide drug information and therapeutic guidance to physicians, nurses, and other healthcare providers.
- Assess medication appropriateness, including renal and hepatic dosing adjustments, drug interactions, and compatibility.
- Support diverse patient populations including pediatric, geriatric, and those with complex therapies like IV antibiotics and chemotherapy.
- Document clinical interventions and pharmacy activities accurately and promptly.
- Maintain compliance with HIPAA, pharmacy laws, and client policies.
