Requirements
- 3+ years of experience in healthcare operations, care coordination, or scheduling operations management.
- Experience in home-based care, hospice, or palliative care settings is strongly preferred.
- Demonstrated experience managing, hiring, and training a team of coordinators or schedulers.
- Strong data and reporting skills with the ability to build and maintain recurring metrics reports.
- Proficiency in Microsoft Excel is required.
- Experience managing operational rollouts or expansions across multiple sites or markets.
- Strong relationship management skills with experience serving as a primary point of contact for external partners.
- Ability to resolve operational friction and scheduling conflicts collaboratively.
- Proactive problem-solving skills to address recurring operational issues.
- AI skills recommended.
Responsibilities
- Manage the team responsible for patient outreach, warm handoffs from referral partners, and referral/screening completion.
- Own live patient scheduling against a shared calendar system, applying market-specific provider rules.
- Forecast staffing needs by market based on referral volume and panel growth.
- Report core central metrics and build recurring reporting cadence for quarterly business reviews.
- Hire, train, and manage the central calling and scheduling staff.
- Build and maintain SOPs for patient outreach, referral completion, and scheduling across markets.
- Serve as the primary operational point of contact for external partner leadership and staff.
- Lead operational onboarding for new market expansions including rule configuration and staffing ramp.
- Coordinate with market-based clinical teams to resolve scheduling logistics and issues.
