Requirements
- At least 2 years of experience in behavioral health or social services.
- Bachelor's degree in behavioral health or human services (e.g., psychology, social work, counseling) or equivalent experience.
- At least 2 years of experience using Microsoft Office applications including Word, Excel, and Outlook.
- Working knowledge of Microsoft 365 tools (Teams, PowerPoint, etc.).
- Strong critical-thinking, problem-solving, and time-management skills.
- Comfortable working across multiple monitors and digital communication tools.
- Ability to work 8:00 AM–5:00 PM in the assigned market time zone.
- Dedicated, quiet, secure, and private home workspace.
- Hardwired internet connection (min 25 Mbps download/3 Mbps upload); Wi-Fi and satellite are not permitted.
- Case management and discharge-planning experience is preferred.
- Managed-care experience is preferred.
Responsibilities
- Evaluate referred members using care-management tools, available data, and benefit information to understand needs, eligibility, and available resources.
- Review member benefit plans and identify appropriate internal and external programs, services, and support options.
- Identify high-risk factors and escalate cases to clinical case management or crisis intervention when appropriate.
- Coordinate assigned care-plan activities, monitor progress, and follow up to ensure identified needs are addressed.
- Collaborate with case managers, supervisors, Medical Directors, and healthcare programs to overcome barriers.
- Provide members with coaching, education, and practical support to encourage independent healthcare decisions.
- Maintain accurate, timely, and complete documentation of member interactions and care coordination activities.
- Navigate multiple systems and applications while following secure-system and HIPAA-related practices.
