Requirements
- Bachelor's degree or non-licensed master level clinician in behavioral health or human services (psychology, social work, marriage and family therapy, counseling).
- 2 years of experience in behavioral health, social services, or related field.
- Ability to travel within Kankakee, Iroquois, Livingston, Ford, Champaign and surrounding counties for in-person activities.
- Proficiency with MS Word, Excel, Outlook, and PowerPoint.
- Ability to navigate multiple systems and perform efficient keyboarding.
- Excellent analytical and problem-solving skills.
- Effective communication, organizational, and interpersonal skills.
- Confidence working independently from home and connecting with teams virtually.
Responsibilities
- Conduct comprehensive evaluation of member's needs and eligibility using care management tools.
- Recommend approaches to case resolution and meeting member needs.
- Identify high-risk factors and service needs, referring to clinical case management or crisis intervention as needed.
- Coordinate and implement assigned care plan activities and monitor progress.
- Consult with case managers, supervisors, and Medical Directors to overcome barriers to care.
- Utilize negotiation skills to secure services and resources.
- Use motivational interviewing to promote member engagement and healthy lifestyle choices.
- Provide coaching and information to empower members in healthcare decision-making.
