- Experience
- At least three years of clinical RN experience in a medical/surgical or ambulatory care setting.
Requirements
- Active, current, unrestricted Registered Nurse (RN) license in a Nurse Licensure Compact (NLC) state.
- At least three years of clinical RN experience in a medical/surgical or ambulatory care setting.
- Case Management Certification (CCM) required or must be obtained within three years of employment.
- Bachelor’s degree in nursing or a related field is preferred.
- Strong clinical knowledge of healthcare and social-support needs.
- Demonstrated ability to identify care barriers and develop practical solutions.
- Understanding of care coordination, alternative care models, and interdisciplinary collaboration.
- Knowledge of assistive equipment and disability-related legislation such as the Americans with Disabilities Act.
- Strong organizational, prioritization, time-management, and clinical judgment skills.
- Excellent written, verbal, and interpersonal communication skills.
Responsibilities
- Apply core principles of case management including assessment, planning, implementation, coordination, monitoring, and evaluation.
- Conduct comprehensive assessments of medical, social, psychological, and long-term needs.
- Develop individualized plans of care based on clinical assessments, benefit-plan considerations, and cost-effectiveness.
- Identify and address barriers to care by coordinating with rehabilitation facilities, home health services, and community resources.
- Maintain effective working relationships with physicians, providers, payors, and interdisciplinary care teams.
- Ensure documentation compliance with regulatory and privacy requirements.
- Negotiate cost discounts with providers and payors when appropriate.
