Requirements

  • 5+ years of experience as a clinical social worker, care coordinator, care navigator, or in a similar field, with demonstrated progressive program management experience.
  • At least 1 year of experience working remotely, with the ability to work independently from a dedicated home office.
  • Direct experience working with Veterans and/or within the VA system.
  • Strong knowledge of VA healthcare, Veteran benefits and resources, behavioral health, and social determinants of health.
  • Experience supporting medically and/or behaviorally complex populations.
  • Experience engaging with members by phone and text message and documenting case notes within a care management or electronic health record platform.
  • Experience using motivational interviewing techniques to build trust, encourage engagement, and support positive health outcomes.
  • Strong interpersonal, written and verbal communication, collaboration, critical thinking, and problem-solving skills.
  • Ability to work effectively within multicultural environments and demonstrate cultural awareness, curiosity, and continued learning about diverse communities.
  • Ability to influence others toward positive outcomes while maintaining empathy, professionalism, and strong relationship-building skills.
  • Resilience and adaptability when handling challenging member interactions, with a willingness to learn from setbacks and continuously improve.

Responsibilities

  • Support the development and refinement of Veteran-specific care coordination workflows, processes, guidelines, and escalation pathways.
  • Help define appropriate scopes of work for non-clinical care coordinators, nurses, and social workers while identifying program gaps and opportunities for improvement.
  • Serve as a subject matter expert on VA healthcare, Veteran benefits, community resources, and Veteran-specific services.
  • Help care coordinators navigate VA, community, and Veteran-focused resources and provide benefits education when additional assistance is needed.
  • Provide guidance on complex Veteran cases, including recommendations for appropriate referrals, resources, and escalations.
  • Deliver direct Veteran support when needed, helping members navigate healthcare and social needs such as food, housing, and other community resources.
  • Partner with training and care coordination leadership to develop and deliver Veteran-specific training programs.
  • Educate care coordinators on VA healthcare, benefits navigation, Veteran resources, behavioral health considerations, and common barriers to accessing care.
  • Act as an ongoing social work and VA subject matter expert for the care coordination team.
  • Advocate for and motivate members to actively manage their health and healthcare while maintaining positive relationships with members, families, providers, and external partners.
