Requirements
- Active, unrestricted Registered Nurse (RN) license in good standing.
- Nurse Licensure Compact (NLC) multistate license or privilege required where applicable.
- Ability and willingness to obtain and maintain additional state licenses as necessary.
- Bachelor of Science in Nursing (BSN) degree.
- Minimum of five years of clinical nursing experience.
- At least one year of experience in telehealth, virtual care, or ambulatory care.
- Proficiency with electronic health records (EHR) and virtual care technology.
- Strong clinical assessment, critical thinking, and escalation judgment.
- Ability to provide clear, evidence-based member education within the RN scope of practice.
- Excellent verbal and written communication skills.
- Certified Diabetes Care and Education Specialist (CDCES) preferred.
- Two or more years of experience supporting individuals with chronic cardiometabolic conditions preferred.
Responsibilities
- Conduct scheduled telehealth nursing visits including clinical intake, medication reconciliation, and symptom review.
- Provide virtual nursing support and respond to member inquiries regarding symptoms, medications, and disease management.
- Deliver evidence-based education on medication, lifestyle, and chronic disease self-management.
- Identify and communicate acute clinical findings or status changes requiring provider intervention.
- Maintain accurate and timely documentation of encounters in the electronic health record.
