Requirements
- Doctor of Pharmacy (PharmD) degree.
- Current pharmacist license in Washington.
- Completion of a PGY2 Ambulatory Care Residency OR completion of a PGY1 Residency and at least 1 year of ambulatory practice under a CPA.
- Alternatively, at least 3 years of ambulatory practice with disease state management.
- Strong communication, organization, and technical skills.
- Proficiency in Microsoft Office tools including Outlook, Teams, Word, Excel, and PowerPoint.
- Ability to navigate the internet and web-based platforms.
- Must pass a data entry and computer skills assessment.
Responsibilities
- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments.
- Partner with physicians and interdisciplinary teams to order individualized care plans under a CPA.
- Monitor therapy effectiveness and safety, identifying and intervening on medication-related problems.
- Support quality‑improvement and REMS initiatives.
- Provide recommendations to operations by supporting teammates with complex prior authorizations and workflow improvements.
