Requirements
- Completion of an accredited Nurse Practitioner program.
- Active Nurse Practitioner or APRN license in good standing.
- Active licensure in multiple states.
- 2+ years of experience in primary care, telehealth, or care management.
- Bilingual fluency in Mandarin.
- Familiarity with Medicare programs, including care navigation and incident-to requirements.
- Knowledge of SDOH documentation.
- Proficiency with EMRs, care management platforms, and telehealth workflows.
Responsibilities
- Conduct telehealth-based E/M or Annual Wellness Visits to establish medical necessity for care navigation services.
- Identify high-risk conditions and unmet social needs during intake visits.
- Document clinical findings in the platform using accurate coding, SDOH Z-codes, and relevant diagnoses.
- Develop and approve individualized care plans for patients.
- Collaborate with care advocates and nurse care managers to align plans with medical and social needs.
