Requirements
- M.D. or D.O. degree.
- Active, unrestricted U.S. medical license.
- Broad multistate licensure (IMLC participation or 50-state coverage preferred).
- 2+ years of experience in primary care, telehealth, or care management.
- Bilingual Mandarin fluency.
- Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
- Familiarity with Medicare programs, incident-to requirements, and SDOH documentation.
- Proficiency with EMRs, care management platforms, and telehealth workflows.
Responsibilities
- Conduct initiating telehealth visits, including E/M or Annual Wellness Visits, to establish medical necessity.
- Identify high-risk conditions and unmet social needs during patient intakes.
- Document findings in the platform using accurate coding, diagnoses, and SDOH Z-codes.
- Develop and approve individualized care plans.
- Collaborate with advocates and clinical operations to ensure safe and compliant patient enrollment.
