Requirements
- M.D. or D.O. degree.
- Active, unrestricted U.S. medical license.
- Broad multistate licensure (IMLC participation or 50-state coverage preferred).
- Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
- 2+ years of experience in primary care, telehealth, or care management.
- Familiarity with Medicare programs, including care navigation and incident-to requirements.
- Experience with SDOH documentation.
- Proficiency with EMRs, care management platforms, and telehealth workflows.
- Bilingual proficiency in Spanish.
Responsibilities
- Conduct telehealth-based E/M or Annual Wellness Visits (AWVs) to establish medical necessity.
- Identify high-risk conditions and unmet social needs.
- Document findings, including SDOH Z-codes, diagnoses, and risk factors, within the platform.
- Develop and approve individualized care plans for patients.
- Ensure alignment between patient care plans and their broader medical treatment plans.
- Collaborate with advocates and clinical operations to ensure safe, compliant enrollment in care navigation programs.
