Requirements
- Completion of an accredited Nurse Practitioner program.
- Active Nurse Practitioner or APRN license in good standing.
- Licensed in multiple U.S. states.
- 2+ years of experience in primary care, telehealth, or care management.
- Bilingual Spanish proficiency.
- Familiarity with Medicare programs, including care navigation and incident-to requirements.
- Proficiency with SDOH documentation.
- Proficiency with EMRs, care management platforms, and telehealth workflows.
Responsibilities
- Conduct telehealth-based E/M or Annual Wellness Visits to establish medical necessity.
- Identify high-risk conditions and unmet social needs during intake consultations.
- Document findings in the platform, including accurate coding and SDOH Z-codes.
- Develop and approve individualized care plans.
- Collaborate with care advocates and nurse care managers to ensure alignment with patient needs.
