Experience
Completion of a PGY2 Oncology Residency; or completion of PGY1 Ambulatory Care Residency and 3 years of experience as a clinical oncology pharmacist; or 3 years of experience as a clinical pharmacist in oncology, including 1 year practicing under a CPA.

Requirements

  • Doctor of Pharmacy (PharmD) degree.
  • Valid pharmacist license in California.
  • Completion of a PGY2 Oncology Residency; OR
  • Completion of PGY1 Ambulatory Care Residency and at least 3 years of experience as a clinical oncology pharmacist; OR
  • At least 3 years of experience as a clinical pharmacist in oncology, including 1 year practicing under a Collaborative Practice Agreement (CPA).
  • Proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint).
  • Strong communication, organization, and technical skills.
  • Ability to pass a pre-employment data entry and computer skills assessment.

Responsibilities

  • Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments.
  • Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA.
  • Promote seamless medication coordination and monitor therapy effectiveness and safety.
  • Identify optimization opportunities and intervene as appropriate.
  • Support quality‑improvement and REMS initiatives.
  • Provide recommendations to operations by supporting teammates with complex prior authorizations and improving workflows.
  • Educate patients and maintain ongoing clinical development.
