- Experience
- Completion of a PGY2 Oncology Residency; or completion of PGY1 Ambulatory Care Residency and 3 years of experience as a clinical oncology pharmacist; or 3 years of experience as a clinical pharmacist in oncology, including 1 year practicing under a CPA.

Requirements
- Doctor of Pharmacy (PharmD) degree.
- Valid pharmacist license in California.
- Completion of a PGY2 Oncology Residency; OR
- Completion of PGY1 Ambulatory Care Residency and at least 3 years of experience as a clinical oncology pharmacist; OR
- At least 3 years of experience as a clinical pharmacist in oncology, including 1 year practicing under a Collaborative Practice Agreement (CPA).
- Proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint).
- Strong communication, organization, and technical skills.
- Ability to pass a pre-employment data entry and computer skills assessment.
Responsibilities
- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments.
- Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA.
- Promote seamless medication coordination and monitor therapy effectiveness and safety.
- Identify optimization opportunities and intervene as appropriate.
- Support quality‑improvement and REMS initiatives.
- Provide recommendations to operations by supporting teammates with complex prior authorizations and improving workflows.
- Educate patients and maintain ongoing clinical development.
