Board Certified as a Family Nurse Practitioner
Active Family Nurse Practitioner License in Michigan
Technologically savvy and comfortable using laptops or mobile devices
Experience using HIPAA secure messaging apps for care coordination
Strong written and verbal communication skills
Ability to work independently or in a team environment
Pass national background check and valid clinical license search
Commitment to professionalism, integrity, and accountability
Experience performing wellness visits
Proven problem-solving abilities and willingness to work in a dynamic environment

Responsibilities

Review patient caseloads and prepare necessary technology for virtual service delivery.
Navigate daily schedules and partner with in-home clinical 'Sprinters' to support patient visits.
Perform health risk assessments, medication reviews, and cognitive screenings.
Educate patients on their health and well-being during virtual wellness exams.
Collaborate with in-home clinical teams to evaluate vital signs, order blood draws, and address specific patient needs.
Provide comprehensive care coordination including preventive interventions, specialist referrals, and documentation.
