Board Certified as a Family Nurse Practitioner.
Active Family Nurse Practitioner license in Indiana.
Experience performing wellness visits.
Technologically savvy with proficiency in laptops, mobile devices, and charting tools.
Experience using HIPAA-secure messaging apps for care coordination.
Strong written and verbal communication skills.
Ability to work independently and as part of a team.
Demonstrated professionalism, integrity, and accountability.
Willingness to engage in problem-solving in a fast-paced environment.
Must pass national background check and clinical license search.

Responsibilities

Review daily patient caseload and prepare necessary technology tools.
Navigate daily schedules using digital platforms and partner with in-home clinical Sprinters.
Establish welcoming connections with patients to collect information and perform services.
Conduct thorough health risk assessments, medication reviews, and cognitive screenings.
Provide patient education regarding health and well-being.
Collaborate with in-home teams to evaluate vital signs and arrange blood draws.
Manage comprehensive care coordination including preventive interventions and specialist referrals.
