Experience performing wellness visits.

Responsibilities

Review caseloads and prepare technology for virtual patient visits.
Navigate daily schedules and partner with in-home clinical team members (Sprinters).
Establish connections with patients to collect information and perform services.
Conduct thorough health risk assessments, medication reviews, and cognitive screenings.
Empower patients through education on health and well-being.
Collaborate with Sprinters to evaluate vitals, arrange blood draws, and address patient needs.
Provide comprehensive care coordination, including preventive interventions, referrals, and documentation.
