Requirements
- Board Certified as a Family Nurse Practitioner.
- Active Family Nurse Practitioner License in Connecticut.
- Proficiency with laptops and mobile devices for charting.
- Experience using HIPAA-secure messaging apps for care coordination.
- Ability to perform wellness visits independently.
- Strong written and verbal communication skills.
- Ability to work autonomously and in a team environment.
- Willingness to engage in problem-solving in a fast-paced environment.
- Maintain professionalism, integrity, and accountability in all patient interactions.
- Pass national background check and valid clinical license search.
Responsibilities
- Review patient caseload and prepare technology and tools for virtual visits.
- Partner with clinical in-home team members to coordinate care.
- Conduct thorough health risk assessments, medication reviews, and cognitive screenings.
- Empower patients with educational information regarding health and well-being.
- Evaluate vital signs, arrange blood draws, and address specific patient needs.
- Provide comprehensive care coordination, including preventive interventions and specialist referrals.
- Document all patient findings accurately.
