Requirements
- 1+ year of experience working in a metric or quota-driven environment
- Bilingual (English/Spanish) with proficiency in reading, writing, speaking, and listening
- Physically located in the United States
- Access to a quiet, HIPAA compliant and internet connected space
- Strong technical skills and comfort with utilizing multiple technology platforms
- Valid driver’s license and auto liability insurance
- Dedicated to working within and improving the local community
- Results driven with a proven track record of exceeding performance indicators
Responsibilities
- Identify, outreach, engage and enroll individuals into the program virtually through phone, text and email, and in-person at clinic, community based organization, and health system partner locations
- Listen openly to eligible members and meet them where they are to build trust and facilitate enrollment
- Offer education to partners regarding Pair Team’s services to link members to Enhanced Care Management
- Explain services and benefits of Enhanced Care Management to hesitant or distrusting individuals
- Promote effective and timely communication amongst internal teams and external partners
- Meet key enrollment metrics and manage the end-to-end enrollment funnel
- Provide consistent updates on funnel stages, delays, and blockers
