Requirements
- High school diploma or GED required.
- Active Washington State Substance Use Disorder Professional (SUDP) credential required.
- Minimum of six months of experience working in an addictions treatment or substance use setting.
- Experience providing case management, behavioral health support, or community-based services preferred.
- Strong interpersonal skills with the ability to build trusting relationships with clients.
- Ability to work collaboratively within a multidisciplinary care team.
- Commitment to culturally responsive, inclusive, and person-centered services.
- Ability and willingness to use a personal vehicle for work-related transportation.
- Valid personal vehicle insurance with the candidate listed as a driver.
- Current vehicle registration and ability to pass required vehicle safety inspection.
- Strong organizational skills and ability to manage multiple client needs effectively.
Responsibilities
- Serve as a primary support and case manager for individuals experiencing mental health conditions and substance use challenges.
- Provide community-based services focused on recovery, rehabilitation, harm reduction, and long-term stability.
- Assist clients with developing and achieving individualized recovery goals through person-centered care planning.
- Coordinate services and connect clients with appropriate community resources, healthcare providers, and support networks.
- Support clients in improving daily living skills, independence, social connections, and overall wellness.
- Collaborate with multidisciplinary teams to provide comprehensive behavioral health support.
- Maintain accurate documentation, case notes, and service records according to program standards.
- Provide ongoing guidance, encouragement, and advocacy to individuals navigating recovery journeys.
- Transport clients when required using a personal vehicle that meets safety and insurance requirements.
- Participate in scheduled shifts, including occasional evenings, weekends, or holidays based on program needs.
