Requirements
- Bachelor’s degree (Human Services, Social Work, Criminal Justice, Healthcare Administration, or related field preferred).
- Proficiency in Google Workspace, Microsoft Excel/Sheets, and data analytics or referral tracking tools.
- Prior experience navigating Electronic Health Record (EHR) systems in a HIPAA-compliant environment.
- Demonstrated knowledge of or interest in justice-involved populations and re-entry initiatives.
- Superior organizational and multi-tasking skills with experience in high-volume scheduling.
- Exceptional verbal and written communication skills.
- Ability to provide empathetic, trauma-informed customer service.
- Ability to thrive in a fast-paced, high-urgency remote environment.
Responsibilities
- Receive, review, and process incoming referrals across 37 Washington justice collaborations.
- Attend and participate in warm hand-off case management meetings with justice partners and care teams.
- Schedule member intake assessments and follow-up appointments to reduce no-show rates.
- Collaborate with internal care teams and external partners to maintain clear communication workflows.
- Handle high-volume communication with members and stakeholders with empathy and professionalism.
- Maintain accurate referral logs and enrollment metrics in Electronic Health Record systems.
- Update a centralized database of community resources for justice-impacted populations.
