Requirements

Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the coverage area.
Board certification (required).
Ability to travel locally and within licensed states as needed.
Proficiency with tablet-based (iPad) clinical workflows.
Ability to work in diverse private home environments.
Capacity to comfortably work around household pets, secondhand smoke, or other common household allergens.

Responsibilities

Conduct comprehensive in-home health evaluations and virtual visits.
Perform medication and medical history reviews.
Execute diagnostic tests such as diabetic eye exams, spirometry, and bone density screenings.
Provide patient education to improve health outcomes and understanding of health conditions.
Travel locally and within licensed states as needed.
Build trusted one-on-one relationships with health plan members.
