Requirements
- Active, unrestricted license as a Nurse Practitioner or Physician Assistant in the coverage area.
- Board certification (required).
- Ability to travel locally and within licensed states.
- Comfortably and safely perform duties in diverse home environments, including those with pets or allergens.
- Proficiency with iPad and clinical documentation workflows.
- Multi-state licenses (preferred).
- Availability for 40 hours per week.
Responsibilities
- Provide In-Home Health Evaluations to members in their residences.
- Conduct comprehensive medication and medical history reviews.
- Perform physical evaluations and necessary diagnostic tests such as diabetic eye exams, spirometry, and bone density screenings.
- Bridge gaps in care by providing education and addressing health questions.
- Utilize provided iPad and clinical workflow for documentation.
- Travel locally and within licensed states as required.
- Conduct virtual visits as needed.
