Requirements
- Certified Nurse Practitioner in Adult, Family or Geriatrics.
- Active state licensure in NJ.
- 5+ years of post-graduate clinical experience in Primary Care or Geriatrics (2 years may be substituted by SNF or LTC facility experience).
- Direct experience engaging Medicare and Medicaid populations.
- Experience managing complex, comorbid conditions.
- Proficiency in motivational interviewing techniques.
- Comfort working in a digital-first environment with experience or interest in virtual care.
- Solid tech proficiency with familiarity in Google Suite and Slack.
- Excellent written and verbal communication skills.
Responsibilities
- Conduct telephonic outreach and scheduled video/phone visits for post-hospital discharge patients.
- Perform medication reconciliation, symptom screening, and order necessary follow-up labs, medications, or supplies.
- Build patient rapport to explain the care model and connect patients to required services.
- Coordinate care across hospital, emergency departments, pharmacies, and community providers.
- Collaborate with nursing teams to meet individual and team-level population health performance targets.
