  • Strong documentation and care coordination skills

Responsibilities

  • Build and maintain ongoing, trust-based relationships with patients.
  • Screen and assess patients for behavioral health symptoms.
  • Collaborate with the care team to develop and adjust treatment plans.
  • Use evidence-based approaches like behavioral activation and motivational interviewing.
  • Monitor progress using validated tools like PHQ-9 and GAD-7.
  • Serve as a liaison between patients, primary care, and psychiatric consultants.
  • Participate in caseload consultations.
  • Maintain accurate, timely, and audit-ready documentation.
  • Partner with teams to improve clinical workflows.
