Responsibilities
- Support quality improvement initiatives focused on improving outcomes, compliance, and performance within End-Stage Renal Disease (ESRD) programs and healthcare settings
- Partner with providers, facilities, and internal teams to analyze performance data, identify opportunities for improvement, and facilitate implementation of evidence-based quality improvement strategies
- Serve as a clinical resource and advisor, helping drive measurable improvements in patient care, regulatory compliance, and healthcare quality outcomes
- Assess complex needs, develop person-centered care plans, and connect individuals to critical resources that improve health outcomes and quality of life
- Provide subject matter expertise related to hospitalization, vaccination, patient and family engagement, and mental health/depression screening
- Support the organization’s mission through advocacy, education, and outreach across diverse and underserved populations
