Requirements

  • Active, unencumbered CMA or RMA certification.
  • Current Basic Life Support (BLS) certification.
  • Proven experience in a clinical or care coordination setting.
  • Strong proficiency in EHR systems and virtual documentation.
  • Excellent verbal and written communication skills with the ability to explain complex health information clearly.
  • A dedicated, HIPAA-compliant, and distraction-free home office environment with high-speed internet.
  • Ability to work independently and manage a structured daily workflow without direct supervision.
  • Prior experience specifically in Chronic Care Management (CCM) or Transitional Care Management.
  • Familiarity with navigating community resources to support patient social determinants of health.
  • Experience working with quality metrics and CMS compliance standards.
  • Demonstrated success in high-volume, fast-paced remote healthcare environments.

Responsibilities

  • Serve as a vital link between patients managing long-term health conditions and their clinical care teams.
  • Provide essential support by conducting monthly outreach, helping patients follow their care plans, and monitoring health data remotely.
  • Manage a schedule of outreach calls to patients.
  • Conduct health risk assessments.
  • Document clinical interactions in our EHR system.
  • Assist with Remote Patient Monitoring (RPM) tasks.
  • Act as a primary point of contact for patient questions and medication adherence support.
  • Ensure patients feel supported, informed, and engaged in their treatment plans.
  • Build trust with patients and accurately document health interactions.
  • Provide clear communication to the broader care team.
