Requirements
- Certified Nurse Practitioner (NP) licensed to practice in at least two of Maryland, D.C., Virginia
- Medicare and Medicaid enrollment
- DEA Number
- Minimum of 3 years of experience as an NP in cardiovascular care or related fields
- Outpatient clinic experience (ideally in cardiology)
- Strong understanding of telehealth platforms and remote patient monitoring
- Excellent communication and interpersonal skills
- Ability to work independently in a remote environment
- Ability to travel to partner cardiology clinics at least twice per month
- Ability to travel daily in the first 4 weeks for training
- Prior experience in telehealth or remote care delivery
- Familiarity with advanced workflows and remote healthcare technology systems
- Dual licensing in multiple states is highly desirable
Responsibilities
- Provide remote cardiovascular care to patients on behalf of clients
- Collaborate closely with cardiologists and clinical teams
- Deliver telehealth services to manage low-acuity cardiovascular disease patients across the nation
- Provide virtual patient assessments, coordinate care, and monitor patient progress remotely
- Collaborate with in-office clinicians and staff at client practices to ensure continuity of care
- Utilize technology platforms and workflows to optimize patient care delivery and streamline workloads
- Ensure timely and accurate documentation of patient encounters in electronic medical records (EMRs)
- Educate patients and their families on cardiovascular conditions and lifestyle modifications
- Participate in ongoing training and continuing medical education (CME)
