- Active CMA/RMA certification (nationally recognized)
- Active BLS certification
- Strong organizational skills
- Ability to work independently
- Sound judgment
- Excellent problem-solving, clinical reasoning, and critical thinking skills
- Strong verbal and written communication skills
- High proficiency working in EHR systems
- Proficient in Google Suite
- Proficient in EMR systems
- Bilingual (English & Spanish)
- HIPAA-compliant workspace free of distractions
- A private room with a locked door to prevent accidental PHI disclosures
- High-speed internet and a CCS-approved computer with two monitors
Responsibilities
- Conduct CCM verbal enrollments and determine patient eligibility
- Obtain and document verbal consent for CCM services
- Educate patients and families on chronic conditions, medications, and care expectations
- Communicate effectively with providers, staff, and healthcare professionals
- Promote adherence to individualized care plans
- Increase patient self-management, assist with shared decision-making, and support patients in reaching established goals
- Connect patients to relevant community resources to improve outcomes and satisfaction
- Ensure compliance with CMS guidelines and healthcare regulations
