Requirements:
  • Native or near-native fluency in both English and Spanish (written and spoken) is mandatory to effectively communicate with diverse client populations and stakeholders.
  • Background as a Medical Doctor (MD), Registered Nurse (RN), or other relevant healthcare professional is a fundamental requirement for this role.
  • Must possess a reliable personal computer with a Windows operating system and a stable, high-speed internet connection.
  • Demonstrated strong written and spoken English language skills; proficiency in Spanish is a critical requirement.
  • Highly organized and detail-oriented with proven experience accurately completing government or service-based applications.
  • Proficient in utilizing various online platforms and case management CRM systems.
  • Exceptional communication and interpersonal skills, coupled with a strong commitment to client advocacy.
  • Comprehensive understanding of the complexities of housing instability, the principles of trauma-informed care, and the impact of social determinants of health.
Responsibilities:
  • Conduct comprehensive triage of urgent client situations to accurately assess immediate needs related to housing, safety, and health.
  • Coordinate move-in logistics and short-term emergency assistance.
  • Proactively scout and identify suitable housing options, including transitional, temporary, and permanent units, based on client needs and availability.
  • Provide comprehensive support to clients by completing and submitting complex housing applications accurately and efficiently.
  • Coordinate essential move-in logistics and facilitate access to short-term emergency assistance as required.
  • Serve as a dedicated advocate for clients, effectively communicating with housing agencies, landlords, and service providers to ensure timely access to resources and equitable treatment.
  • Facilitate seamless referrals to crucial care coordination, mental health services, medical care, and relevant community support programs.
  • Maintain meticulous and up-to-date case documentation, including detailed tracking of referrals, interventions, and client outcomes, adhering to established protocols.
  • Collaborate effectively with U.S.-based teams, ensuring seamless communication and adherence to all HIPAA regulations and confidentiality protocols.
