Requirements:

• Maintain a minimum of 5 licenses in states where Theoria actively provides care.
• Experience in primary care services.
• Experience working with patients in SNFs.
• Timely documentation of all patient care/encounters.

Responsibilities:

• Maintain a minimum of 5 licenses in states where Theoria actively provides care.
• Manage the clinical care of the population at the assigned SNFs.
• Monitor message boards, answering calls, and communicating with assigned SNFs during shifts.
• Provide high-quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
• Be responsible for timely documentation of all patient care/encounters
• Be available for phone/secure messaging consultations with facility nursing staff.
