Requirements
- High school diploma.
- Management diploma or certificate preferred.
- At least 2 years of case or care management experience in a medical group, IPA, or HMO setting preferred.
- Knowledge of NCQA standards, Model of Care, and case management requirements.
- Knowledge of CMS and state regulations and HMO/ACO model structures.
- Strong communication, leadership, and critical-thinking skills.
- Ability to work cross-functionally with clinicians, leadership, and administrative staff.
- Experience managing diverse teams and leading quality-focused initiatives.
- Skilled in Microsoft Office and able to learn and navigate other computer systems.
Responsibilities
- Provide day-to-day supervision of care management teams and ensure adherence to policies, procedures, and regulatory requirements.
- Monitor implementation of the care management process, including staff training and documentation requirements.
- Review health plan reports to identify members requiring care coordination or targeted interventions.
- Conduct internal audits of ambulatory care management cases and provide staff feedback and coaching.
- Educate staff on documentation standards, best practices, and quality improvement opportunities.
- Support submissions of deliverables for health plan audits.
- Conduct performance evaluations and productivity audits, and monitor staff attendance.
- Oversee hiring, onboarding, training, timekeeping, and personnel records for staff.
