Requirements
- High school diploma or equivalent (GED).
- Minimum of 1 year of related work experience.
- Certified Community Health Worker (CCHW) certification from the Pennsylvania Certification Board.
- Skills in clerical work, communication, multitasking, and problem solving.
Responsibilities
- Assess new Community Health Workers and evaluate staff proficiency during orientation.
- Identify and conduct outreach to medically complex patients.
- Collaborate with the multidisciplinary team to develop and implement care plans.
- Engage patients by phone, home visits, hospital visits, and accompaniment to provider appointments as needed.
- Complete vital signs and conduct safety evaluations in patients' homes.
- Identify gaps in care and connect members with resources.
- Facilitate telemedicine visits with members and providers.
- Provide disease-specific education and conduct medication reviews to support adherence.
- Communicate concerns to the clinical team and manager, and participate in multidisciplinary team conferences.
- Reinforce treatment plans under the direction of the Case Manager and Licensed Social Worker.
