Requirements
- Hold current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
- Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Work independently on day-to-day activities.
- Communicate by phone, email, and in writing.
- Manage visits, documentation, and meetings, and respond to urgent needs by prioritizing and triaging multiple concerns.
- Understand hospice rules and regulations.
- Be able to learn an electronic medical records system.
- Be willing to drive across the service area with reliable transportation, a valid driver’s license, and auto insurance.
Responsibilities
- Provide care and services for a group of patients.
- Visit patients where they reside, including private homes, skilled nursing facilities, and assisted living facilities.
- Contribute expertise in psychosocial assessments and interventions to the interdisciplinary team.
- Apply knowledge of community resources.
- Work in a case management, admissions, or resource role.
- Coordinate with nurses, social workers, chaplains, volunteers, and CNA leadership.
